I, as parent or guardian of the above child I do hereby grant permission for my child named above to participate in cheerleading with BSYC. In order that my child may receive the necessary medical treatment in the event they may sustain injury or illness during participating in this activity, I hereby authorize the coach or supervising adult to provide and/obtain medical treatment for my child for such injury or illness during the activity, and I hereby hold the BSYC and its representative harmless in the exercise of this authority.
I understand that this activity involves risk to the participant. I further acknowledge and understand that due to the nature of this activity, there is a possibility that my child may sustain physical injury minimal, serious or catastrophic (including brain injury, paralysis or death), in connection with her participation. If I observe any unusual or significant hazard during my presence or my child’s participation, I will remove myself and my child from participating and I will bring the hazard to the attention of a BSYC representative. I further acknowledge and understand that my child is assuming the risk of physical injury by their participation in this activity to the fullest extent permitted by law.
I further understand the BSYC has established rules and regulations pertaining to conduct, behavior and activities of all cheerleaders, adults and, spectators by which my child and I must abide by during participation in this activity, and that I and my child will be responsible for our failure to abide by those rules and regulations.
I understand that I am financially responsible for equipment/uniform supplied to my child by BSYC. Failure to maintain and return such equipment will result in a financial restitution to BSYC by me for all damage and or loss.
I have read and agree with the terms of BSYC policy as stated above.
