CONSENT & RELEASE
I hereby give Bay Shore Youth Cheer, the right and permission, with respect to the photography, film, video or audio footage that Bay Shore Youth Cheer has taken of my child or in which they may be included with others for the use of Bay Shore Youth Cheer marketing materials, website and social media posts. I hereby release Bay Shore Youth Cheer and their legal representatives, licensees or assigns from any and all claims and demands arising out of or in connection with the use of the photography, film, video or audio footage, including any and all claims for libel. I am over the age of twenty-one and am giving consent as my child’s parent and/or guardian. I have read the foregoing and fully understand the contents thereof.
LIABILITY RELEASE
I release and discharge Bay Shore Youth Cheer from any and all claims and demands arising out of or in connection with the use of these photographs, including but not limited to any claims for invasion of privacy, defamation, or misrepresentation.
